一病人的consultation note
I was asked by Dr XXto evaluate this patient for a new HIV positive diagnosis. this is a 53year old African male patient whose history was difficult to obtain because of French speaking dialect from the Ivory coasst African; even with a french speaking nurse, history was difficult. From what I can tell from the history through interpretation and from the current history in the chart, this gentleman hsa been in Canada, immigrated iaround 2008, finished his schooling in Canada and is currently working as a teacher at a shool. he has a wife in africa and is married, he is unaware of her HIV positive status. this patient was initially admitted to the hospital stating that he hasonly been ill for about a week, complaining of weight loss, malaise, some diffuse itchiness on his skin, and was admitted through the emergency room with a hemoglobin of 77, and was being worked up for causes, and his IV serology returned positive with a positive HIV screen, nd positive HIV-1 western blot done on May the 28th 2012 |
