如何写解释信

我在5月底有一份新的工作，不想为这份工作做合同公证和推荐信了，请问该如何写解释信？
同时如何解释表格里这些内容的变化？

就告诉使馆是新的工作，还不方便提供推荐信和合同就行了。表格更新一下，随说明寄给使馆就行。

To whom it may concern:
I am writing this letter to explain some changes in my forms.
First of all, I changed the home address recently so I update the address of related forms.
At the same time, I have a new job position of a Consulting company and I updated the work experience. For I am in the probation period, the contract and recommend letter can not be provided right now.
