a. 去好的打印店用好纸打印（我们用的是90克的纸打印的）, 表格的空位比较小，因为这个FINGERPRINT CARD是图片格式，不能在电脑上填写，所以我们是找人用老式打字机，直接把内容打上去的。填好后，可复印几份。
b. 有些栏目不用填，到底填不填，如何填，我们也是费了很大劲付费咨询了几家中介公司才知道的。
C. 最上面的lastname, first name要填写（不是签名）。 signature of person fingerprinted: 申请人签名。
d. residence of person fingerprinted: 自己的住址
e. date 和 signature of official taking fingerprints 不用填，空着，等你在公安局或公证处打好指摸后，再由公安局或公证处签字（签汉语拼音）盖章。
f. employer and address: 填上自己现在的单位英文地址
g. Reason fingerprinted: Immigrate to Canada
h. Aliases AKA: 不填
I. Citizenship CTZ: CHINESE
J. YOUR NO OCA： 不填
k. FBI NO FBI：不填(办完无犯罪记录后，FBI 会给你一个number，下次办理FBI有关手续时，才填)。
l. ARMED FORCES NO. MNU:不填
m.SOCIAL SECURITY NO.SOC:如有要填。
n.DATE OF BIRTH DOB:要填。
o.SEX:女人填F，男的填M。
p.RACE:汉族就填HAN。
q.HGT：XXXcm。
r.WET：XXXkg。
s.EYES和HAIR：两个格就填一个BLACK可以了。也有人EYES填BROWN的。HAIR填BLACK的，都没关系。
t.PLACE OF BIRTH POB: 填省（例如guangdong）其他全部都空，这个fingerprint card就 ok了
6．然后把自己复印的那份指摸卡去翻译公司翻译公安局的盖章，盖翻译公司的章就OK了。 译文如下：
“The content of the seal in Chinese on the attached document: The special seal of verification of criminal technology of Guangzhou Public Security Bureau of Guangdong Province”
I am applying for immigration visa to Canada. I have ever studied in the University of XXX
in XXX from XXX to XXX, so I was required to present my No Criminal Conviction from the US within 90 days from XXX by Canadian Consulate.
Enclosed are my documents for requesting a copy of record per your request:
1.Complete cover letter;
2.My 10-finger fingerprints cards with notarized stamp translation.
3.USD 18 made in Bank Draft---payable to The Treasury of the United States;

想再请教两个问题
1。 指模做起来难度很大吗？一定要公安局或者公证处打吗？好像看官方只是说“If possiblehave your fingerprints taken by a fingerprinting technician (this servicemay be available at a Law Enforcement Agency.)”。那么我是否可以理解为，自己做也是可以的？（我人在上海，不知道这里哪里能打指模。）
2. 看您的描述，从您寄出request到收到FBI的证明，大约用了一个半月，是这样吗？那么FBI寄回的时候是用的平信吗？有没有什么方法让他们用快递寄回？
