补料信中提到：

• If you reside in the People’s Republic of China or Taiwan: Hukou (household register) – must be accompanied by a notarized translation into either English or French.  Translation must be part of the notarized booklet (if not already submitted)
• For any dependent children: notarized copy of the original hospital birth certificate.  For children born in the People’s Republic of China: You must submit a notarized copy of the certificate issued by the PRC Ministry of Health (If this is not available, you must submit an explanation as to why this document cannot be submitted).  Kinship certificates are not acceptable.

就是户口的公正书吧？没说copy，所以有点不确定是不是要原件。

地址更新过了，我想应该要更新IMM008表， 这个需要再validate一次吗？条码应该会重新生成的和当初签收材料时的不一样了，要紧不？谢谢。

不更新不要紧，寄护照取签时候会要你提供最新地址标签的
另外你可以在那个电子咨询系统上把你的新地址发过去
