91收到退款确认邮件,表格超复杂,请帮忙

其中有两项不太明白（下面表格里涂成黄色部分）：
1、 Bank Code (i.e. ABA, sort code,BSB etc.) 建设银行的该怎么填写呢？
2、我的是活期储蓄卡，帐户类别 应该是Savings 吧。
我问了银行营业厅，他们也不清楚。业务水平堪忧啊~~
