让朋友带未成年小孩回国（没有父母陪同）情况下，需要填写的授权书样本

前阵子，亲戚的小孩放暑假，而亲戚因为上班不能照看他们，所以决定让朋友把小孩送回国。亲戚找我帮忙写一份《无父母陪同小孩旅行授权》，我上网搜索了一番终于弄出了一份，不敢独享，现拿出来给大家参考，希望有这种情况的朋友能用得上。

简单介绍：

1、什么情况需要这份授权书：
如果小孩（18岁以下）坐飞机，陪同的人不是自己的父母或法定监护人，这个陪同的人需要出示这份授权书，才能获得航空公司的准许带同该小孩一起飞行，不然的话，小孩是不能登记的；

2、授权书的内容是什么：
授权书需要明确小孩的个人资料（姓名、出生日期、性别、护照号码、居住地址等），父母或法定监护人的个人资料，陪同人的个人资料，此次旅行的时段，父母或法定监护人允许陪同人携带自己的小孩完成旅程并负责小孩的安全和健康；

3、授权书需要哪些人签字：
父母或法定监护人需要签字，还需要有公证人的签章（您需要把打印好的授权书拿到公证处让公证人公证）；

Our children, Howard Liu and Helen Liu, have our consent to travel with Biao Shu Liu of 2000 East 18th Ave. Vancouver, B.C. V5S 1F9, Telephone (604) 781-xxxx to visit China. My children will be leaving Canada on or about July 8, 2007, and returning to Canada in or about August 2007.

In the event that our children require emergency medical treatment and we cannot be reached, Biao Shu Liu is authorized to consent to medical treatment in our stead.
